y _ <br />:i <br /> Issued: 6/18/87 <br /> MICRO SDP-P21 <br /> Laborst-ory Custody Procedures <br /> o Samples are received end handled in the Microbiology Group according to <br /> Microbiology Procedure SOP-PO6, "Receiving Test Samples." <br /> o The person in the Microbiology Group receiving the sample is the <br /> custodian of that sample. <br /> a The custodian will indicate receipt by signing the Chain of Custody <br /> Form accompanying the samples and retaining the Form as a permanent <br /> record in a three-ring notebook. <br /> o The custodian will ensure that microbiological samples are properly <br /> stored and maintained. <br /> o The custodian or other gvrlified laboratory person performing the <br /> analyses will record information in the appropriate lab notebook <br /> describing the sample, the procedures performed and the test results. <br /> Procedures for notebook entries will follow the "Rules for Recording <br /> Haskell Laboratory Notebooks" found in the beginning of Haskell <br /> notebook's and Good Laboratory Practices (GLP). <br /> o Once the sample has been tested, the custodian will record the date and <br /> whether or not the sample was disposed of or 'returned to the customer. <br /> r Microbiological waste disposal practices are presented in the <br /> Stine-Haskell Safety and Occupational Heolth Manual, Section II, <br /> Subpart 9' "Procedures for Waste Disposal" and the Environmental <br /> k ' Toxicology's addendum "Section Specific Supplement for Handling Wastes <br /> ¢ from the Environmental Biology Section." <br /> i <br /> 4 <br /> Prepared by: <br /> Carl W.
