Human and primate blood or <br /> blood products must be handled as potentially biohazardous <br /> materials according to the relevant section of the Safety <br /> b Occupational Health Manual. <br /> Procedures: <br /> Sam les Submitted from Outside of Haskell: <br /> o Samples for the minrobiology group arrive at Haskell by <br /> the mail (i.e. directly to study director) or through the <br /> sample room in Shipping and Receiving. The operating <br /> department sending the samples is supplied with green <br /> sample submission forms (see atrachment) by Haskell <br /> personnel. <br /> o Upon arrival check that the sample submission form is <br /> correctly filled out; if not, correct or make out a new <br /> one. <br /> o• Microbiology ("M") numbers are obtained for each sample <br /> written nn the sample submissionform by the Information <br /> Section personnel and photocopied. The Information <br /> Section retains the green Corm and the microbiolcgy group <br /> r places the photocopy in the laboratory- notebook. Yellow <br /> adhesive labels containing the "M" number are provided by <br /> the Information Section and are placed on the samples. <br /> Samples are stota d at room temperature, defrigerated, or <br /> frozen depending on the sample type. <br /> a If excess sample is shipped pped and space needs or disposal <br /> presents a problem, the remaining untested sample is z <br /> returned to the sender. <br /> Sam les Taken by Has!:elj. Personnel: <br /> off-site: <br /> o The appropriately signed shipping/transporatation papers <br /> (form is attached) must accompany any samples transported <br /> between sites.
