SA J O ■ Q U I N Environmental Health Department <br /> A <br /> --- .0 U `.I T'f' <br /> WASTE DISCHARGEISPILL RESPONSE <br /> NOTICE TO ABATE <br /> Responsible Party: GABRIEL CAMPA Date: Jan 10, 2019 <br /> RP Mailing Address: 475 HELEN ROAD Record ID#: COWFKMQJI <br /> Ripon, CA 95366- Program Element 2546 <br /> Spill Location: 23243 E LONE TREE RD, ESCALON Release/Spill Response (excluding Joint Team) <br /> NATURE OF WASTE DISCHARGENVASTE RELEASE: <br /> A car hit a transformer pole caused the spill of transformer oil onto the road way and the shoulder of the road. The Car <br /> ended in a field next to impacted pole and continued to leak vehicle fluid onto stagnant water. Oil sheen appeared on water <br /> surface (approximately 10 x 2 ft area). <br /> OBSERVATIONS: <br /> While inspecting a PG&E transformer oil spill at this location due to a vehicle incident, the vehicle that caused the incident <br /> leaked vehicle fluid into stagnant water in the adjacent field. Patriot Environmental, who conducted the PG&E clean-up <br /> placed two absorbent pads on the top of the contaminated water to catch some of the fluid, and generated waste was taken <br /> with PG&E waste. The following day I inspected the sheen, and noticed some of the sheen still remained on surface of <br /> water, and it was drifted to the west side of the stagnant water. <br /> Per[HSC 5411]: No person shall discharge sewage or other waste, or the effluent of treated sewage or other waste, in any <br /> manner, which will result in contamination, pollution or a nuisance. Please refer to the corrective action/order below to <br /> correct these issues. <br /> VIOLATIONS: <br /> • Discharge of any waste, including sewage <br /> (CA Health and Safety Code 5411) <br /> ° Discharge of waste or sewage to water or within 150 ft. of waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1925.4) <br /> ■ Improper disposal of a hazardous waste <br /> (CA Health and Safety Code 25189.5) <br /> Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> ° Other Violation <br /> CORRECTIVE ACTIONS/ORDER: <br /> • Investigate and abate contamination under the oversite of the appropriate lead agency. <br /> • Submit copies of hazardous waste manifests/disposal records to the EHD. <br /> within 30 days. <br /> • Other Correction <br /> Contact SJC EHD to witness the clean-up. <br /> EHD 25-01 Rev.08/17/17 Page 1 of 2 WASTE DISCHARGE/SPILL RESPONSE NOTICE TO ABATE <br /> 1868 E.
