6. EVACUATION ROUTES AND PROCEDURES <br />A. When to Evacuate: <br />Evacuation of the facility will occur as a response to an incident with known or <br />unknown hazards that could pose a threat to the health and/or safety of facility <br />personnel. <br />The decision to evacuate is the responsibility of the Facility Emergency <br />Coordinator or the shift supervisor. He/she will determine the extent of <br />evacuation. (The local fire department or other emergency response personnel <br />might have the authority to make this decision depending upon the situation.) <br />Incidents that may require evacuation: <br />1. Fire or explosion. <br />2. Hazardous substance spill. <br />3. Flood. <br />4. Bomb threat. <br />5. Any other potentially dangerous situation. <br />[-M"kl B. Evacuation Procedures: <br />• <br />When the signal for evacuation is sounded: <br />1. DO NOT PANIC. <br />2. Shut down any machinery or apparatus. <br />3. Escort/help handicapped or injured personnel to the nearest exit as identified <br />on the facility evacuation map, Figure 6-1. <br />4. Walk, as diagrammed, to the designated assembly point and wait for <br />instructions from the Facility Emergency Coordinator. <br />5. Supervisors must account for all persons in their units and will report any <br />missing persons to Facility Emergency Coordinator. <br />6. The Facility Emergency Coordinator will decide when it is safe to re-enter. <br />M. <br />
