If one set of documents (eg. freight <br />bills and transfer chain -of -custody) covers numerous samples, attach a <br />' sheet of paper listing all the sample numbers. <br />o Attach either ELI form SM -01 or customers own Chain -of -Custody form <br />to analysis order package. Transfer information from SH -01 or from clients <br />chain -of -custody form to SHBK-04 (sample log book). (see Appendix 1, page <br />'59) <br />o Fill out an ELI form SM -02 -Analysis Order/Internal Chain -of -Custody <br />form detailing analysis method and sample information. (see Appendix 1, <br />pages 62 and 63) <br />o Fill out Sample Tag -03 and put on sample container. (see <br />Appendixj, page 65) <br />5.0 Sample Handling and Transportation <br />After a sample is transferred into the proper container, it must be <br />closed as quickly as possible to prevent the loss of volatile components <br />' and to exclude possible oxidation from the air. <br />Provisions must be made to promptly refrigerate those samples <br />requiring preservation reservation at cool temperature (0-4 C). Ice chests are used <br />a <br />1( for this purpose. Samples must also be protected from breakage during <br />transportation or shipment. <br />' A record must be kept of each sample taken. It should be complete <br />enough so that the sample's history may be followed from the time of <br />' sampling to the time the records of the analytical results are stored. <br />0 <br />52 <br />C <br />
