• <br /> Comments: <br /> Routine waste tire inspection done on this date and the following was noted: <br /> 1.)A/C 14 CCR§ 18459- Observed incomplete CTL forms as a hauler and generator. <br /> a.) Manifest Form shall be completed and signed under penalty of perjury by the appropriate representative, and <br /> accompany each shipment of waste or used tires from the point of origin to the facility. <br /> b.) Ensure subsequent manifests are filled out completely. <br /> 2.)A/C 14 CCR§ 18459.2.1 -Search done on WTMS from 12/3/2015 to present resulted in 44 <br /> manifests. Manifests observed on WTMS were on site and available for review. 12 additional manifests were on <br /> site and available for review. The 12 additional manifests reviewed were not submitted to Cal-Recycle within 90 <br /> days of the load shipment. <br /> a.)The waste tire hauler shall submit the completed original CTL Form to Cal-Recycle within 90 days of the load <br /> shipment. <br /> b.) Ensure CTL forms are submitted to Cal-Recycle within 90 days. <br /> Notes: <br /> 1.) Permission to conduct inspection was granted by Mark Hoyle (Equipment Material Manager II). <br /> 2.)TPID certificate is posted in the maintenance office. <br /> 3.)Waste tires were counted directly. <br /> 4.) Retain all waste tire logs, receipts, and manifests for 3 years. <br /> 5.) Facility only stores its waste tires at this location. <br /> 6.) Inspection report emailed to mark_hoyle@dot.ca.gov. <br />
