(Repeat) A current emergency coordinator and modified contingency plan information is lacking. <br /> -- There must be at least one emergency coordinator on site or on call to coordinate emergency <br /> response measures, and the following information must be posted by a phone.. the name and phone <br /> number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if <br /> present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the required information by a phone. <br /> * A form is provided that can be used for this purpose. Violation was corrected on site. <br /> 60 & 63. (Repeat) Copies of hazardous waste disposal records for June 16, 2008 to June 16, 2011 were <br /> not found on site. <br /> -- Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and <br /> all receipts used in a consolidated manifesting procedure on site for three years and have them readily <br /> available for review. <br /> * Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, <br /> and submit copies to the EHD by July 18, 2011. <br /> 79,83,84. Unable to determine the length of storage of three Universal Waste fluorescent lamps not <br /> stored in container, and 4 boxes of Universal Waste lamps and one bucket of Universal Waste alkaline <br /> batteries with no date of start of accumulation. One box of the Universal Waste lamps were not closed. <br /> -- Universal Wastes shall be stored in a closed container that prevents breakage and release to the <br /> environment. The container shall be labeled to identify the type of Universal Waste and marked the <br /> initial date of accumulation. Universal Waste shall be managed within one year of storage and retained <br /> on site a copy of the disposal receipt for at least 3 years. <br /> * Immediately dispose the above Universal Wastes and submit a copy of the disposal receipt to EHD by <br /> July 18, 2011. A pamphlet for the Small Hazardous Waste Disposal Facility and information factsheet <br /> for Universal Waste management have been supplied to the generator during this inspection. <br /> 85.
