(Repeat) A current emergency coordinator and modified contingency plan information is lacking. -- There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone.. the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. * A form is provided that can be used for this purpose. Violation was corrected on site. 60 & 63. (Repeat) Copies of hazardous waste disposal records for June 16, 2008 to June 16, 2011 were not found on site. -- Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. * Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD by July 18, 2011. 79,83,84. Unable to determine the length of storage of three Universal Waste fluorescent lamps not stored in container, and 4 boxes of Universal Waste lamps and one bucket of Universal Waste alkaline batteries with no date of start of accumulation. One box of the Universal Waste lamps were not closed. -- Universal Wastes shall be stored in a closed container that prevents breakage and release to the environment. The container shall be labeled to identify the type of Universal Waste and marked the initial date of accumulation. Universal Waste shall be managed within one year of storage and retained on site a copy of the disposal receipt for at least 3 years. * Immediately dispose the above Universal Wastes and submit a copy of the disposal receipt to EHD by July 18, 2011. A pamphlet for the Small Hazardous Waste Disposal Facility and information factsheet for Universal Waste management have been supplied to the generator during this inspection. 85. Copies of Universal Waste disposal records for 2008 and 2009 were not found on site. -- A copy of the disposal receipts of Universal Waste, including but not limited to lamps, alkaline batteries, mercury containing switches, CRT, and electronic devices, shall be retained on site available for inspection. * Immediately locate a copy of all Universal Waste disposal records for 2008 and 2009, maintain them on site, and submit copies to the EHD by July 18, 2011. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
