COMPLAINANT HAS NO SYMPTOMS OF SICKNESS, BUT <br />UPON WAITING AT TABLE FOR (C) TO GO ORDER, SEEN A ROACH DASH FROM UNDER COUNTER THAT <br />PREPARES FOOD, RUN INTO DINING AREA, THEN RUN BACK UNDER COUNTER. COMPLAINANT POINTED OUT <br />TO CASHIER WHO SAID "WILL GET IT LATER". THEN (C) SEEN THE ROACH AGAIN AND POINTED IT OUT TO <br />CASHIER, WHO CAME OUT FROM AROUND COUNTER AND STEPPED ON ROACH. <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />chicken -- counter warmer (new) -- 149.00º F <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />NOTES <br />I went to the facility and spoke to Person In Charge who was not aware of complaint prior to my visit. <br />I observed kitchen area under counter and did not observe cockroaches at this time. <br />I observed debris on shelf next to cooking equipment and back dry storage area. Clean and sanitize areas. <br />I observed under front counter and did not observe any cockroaches at this time. <br />I observed several miscellaneous items under counter, paper etc. Reorganize remove unnecessary items. <br />I observed storage shelf that has newspaper cookie tins and other items. Reorganize, clean. Remove unnecessary items. <br />Per person in charge pest control services twice per month and also as needed. <br />A follow up to occur in 2 weeks. <br />Per person in charge pest control receipt is not on site, pest control receipt will be available at follow up. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
