Bo STOCKTON CA 95201-0388 5201 <br />TO: INSTALLERS AND OWNERS OF NEW UNDERGROUND STORAGE TANKS <br />San Joaquin County Public Health Services -Environmental Health Division (PHS-EHD) has established the <br />following guidelines to assist you in the completion of your construction project. The guidelines specify the <br />construction and monitoring criteria necessary to complete our review of your proposed underground storage <br />tank (UST) system. <br />1. SUBMITTAL OF PLANS <br />a. Submit 3 complete sets of detailed drawings in non -erasable print to the Environmental <br />Health Division. Upon approval, one set will be retained by the department and the other <br />two will be returned. <br />b. Include equipment and material specification sheets and manufacturer's brochures describing <br />operating and monit` ori�sy§t rld listing numbers or other evidence of a nationally <br />recognized testing organization approval. <br />C. Include detailed monitoring and response plans for facility. <br />d. The drawn to scale plot plan should illustrate the location of all existing and proposed <br />structures, including existing UST(s). <br />e. The plans should include a cross sectional diagram of the tank and piping sysme including <br />secondary containment, overflow protection equipment, and _inextension of all pipes and/or <br />ports from subgrade to finish grade. <br />2. PLAN REVIEW AND OPERATING PERMIT FEES <br />a. New facility plan review fee of $624.00 (8 hour minimum per facility), $170.00 fee per tank, <br />plus 556.00 state surcharge fee per tank is due at time of plan submittal. <br />b. New installation of additional tank at an existing facility is $624.00 plan review fee (8 hour <br />minimum per facility) plus $170.00 fee per new tank, and $56.00 per tank state surcharge fee. <br />C. It should be noted that a $624.00 plan review check fee required for each submittal allows <br />for up to eight (8) hours of staff review and/or inspection time. Additional time spent on <br />plan review, meetings, inspections, phone or office consultations will be billed to the <br />applicant at a rate of $78.00 per hour. <br />d. The maximum review time for UST installation plans is 40 working days. A preliminary <br />review will be conducted by the PHS/EHD within 20 working days of submittal date to <br />identify any gross plan deficiencies. <br />e. If gross deficiencies are identified, the plans will be returned with checklist identifying plan <br />deficiencies. <br />E To expedite the review of installation plans, time and a half per hour overtime fee may be <br />submitted to have review completed within 10 working days of submittal date. <br />A Division of San Jo2qum County Health Care Sernces 10 <br />
