The report of findings for this phase of investigation and any resultant mitigation following <br /> the tank removal shall be submitted to PHS-EHD by April 1, 1994. <br /> If over-excavation of the tank pit area at the time of the tank removal is to be conducted, please make <br /> your intentions known in the closure plan. Additional requirements need to be met in order for this <br /> Corrective Action Plan to proceed. <br /> Monitoring of the three (3) existing monitoring wells currently on site (i.e., depth to water measurements, <br /> hydraulic gradient calculations, and groundwater direction) shall be conducted on a quarterly basis with <br /> reports submitted to PHS-EHD within thirty (30) days of the event. Sampling frequency shall be on a <br /> quarterly schedule also unless a written request to modify it is received and approved by PHS-EHD. <br /> Please include a schedule of the sampling events should you choose to modify the quarterly requirements. <br /> Please direct your tank closure/removal plan and questions to Pam Violett, Senior REHS, at (209) <br /> 468-0335. <br /> Workplans, applications for boring permits, and reports of findings should be directed to Michael Infurna, <br /> Senior REHS. <br /> Ernest M.
