The permit has <br />been approved with the following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br />District for applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. <br />3. At least one person who holds the International Code Council "UST Service <br />Technician" certification must be onsite AT ALL TIMES when work is performed. <br />4. Manufacturer's certification of training presented at time of appointment before any <br />testing can take place. <br />5. Contact Lori Luces at (209) 468-0341 to arrange the following inspection <br />appointments; a 48-hour notice for inspection appointments is required.\ <br />a. Leak Detector Test: leak detector must be tested to confirm that the device <br />detects a leak of 3 gallons per hour at 10 psi. (Leak detector could not be <br />tested previously due to the leak at the dispenser, and must be restested) <br />b. Shear Valve: must be tested to ensure product flow is restricted when device <br />is tripped. <br />6. Permit expires 90 days from the approval date. Note: if an extension to the <br />expiration date is required, this office prior to the expiration date must receive a <br />written request. <br />4 <br />Thank you, <br />Lo J.
