Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by 10-4-12. <br /> 109. Annual monitoring system certification, leak detector testing, and spill container testing were <br /> performed on 1-26-11 and a test report was received by the EHD on 5-16-12, ten months late. A copy of <br /> the test report must be submitted within 30 days of the tests. Take the necessary precautions to ensure <br /> submittal of test results to the EHD within 30 days of the tests. <br /> 109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on 7-26-11 and were due by July 2012. Testing has not been completed and is two <br /> months past due. These tests are required once every 12 months. Immediately schedule these tests <br /> and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be <br /> charged at our current hourly rate. <br /> 204. The designated operator failed to attach the L5 alarm history report on the December 9, 2011, <br /> designated operator monthly inspection report and the L11, L14, and L16 alarm history reports on the <br /> September 14, 2011 designated operator monthly report. During the monthly inspection, the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented <br /> and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br /> response to any alarms to the monthly report. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> 206. Amrit Gill was the only employee on site. She is not on the list of employees trained by the facility <br /> designated operator. At least one of the facility employees present during operating hours shall have <br /> current training in accordance with subdivision (f)(1). Correct immediately by having on site at least one <br /> of the facility employees currently trained in accordance with subdivision (f)(1). The training for facility <br /> employees must include, but is not limited to: (A) The operation of the underground storage tank system <br /> in a manner consistent with the facility's best management practices. (B) The facility employee's role <br /> with the monitoring equipment specified in the facility's monitoring plan. (C) The facility employee's role <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector.
