Current approved copies of the monitoring and response plans were not found on site. The <br /> leak detectors being used are not listed in the monitoring plan. The plan indicates that shutdown occurs <br /> at the dispenser even though it does not. The monitoring and response plans must be current and <br /> approved by the EHD. Immediately log into CERS, update the monitoring plan information, and submit <br /> for review by the EHD. Complete a monitoring plan record for each tank. Immediately submit a copy of <br /> the response plan to the EHD for approval. Alternatively, log into CERS, upload the completed form, <br /> and submit for review by the EHD. Maintain an approved copy on site. <br /> 108. Secondary containment testing was performed on 1-4-13 and a test report was submitted to the <br /> EHD on 2-20-13. A copy of the test report must be submitted within 30 days of the test. Ensure that the <br /> test reports are submitted to the EHD within 30 days of the test. <br /> 109. Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on 9-28-12 and a test report was submitted to the EHD on 1-10-13. A copy of the test report <br /> must be submitted within 30 days of the test. Ensure that the test reports are submitted to the EHD <br /> within 30 days of the test. <br /> 201. The 9-28-12 monitoring system certification test results and maintenance records for the 2-22-13 <br /> sensor alarms were not found on site. These records shall be kept on site for at least 3 years. <br /> Immediately locate and maintain the monitoring system certification test results and monitoring system <br /> sensor alarm maintenance records on site and submit copies to the EHD by October 4, 2013. <br /> 310. Liquid was observed in the diesel turbine sump. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br /> remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br /> and manage it accordingly. Ensure that the secondary containment sumps are maintained free of liquid. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANYTIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector.
