The Veeder <br />Root 0794380-208 sensor serial # 727372 in the 91 fill sump has already been installed and <br />retested to pass during the monitoring system certification on 7-28-10. The EHD has also <br />completed reviewing the addendum to include the replaced shear valve for the diesel product at <br />the #5/6 under dispenser containment. An additional addendum has been added to include the <br />91 annular sensor that was replaced and tested to pass during the 10-13-10 SR0060637 <br />inspection. The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />4. Qualified installers must possess or work under the direct and personal supervision of an <br />individual physically present at the work site who possesses all of the following: <br />a. A valid, current contractor's license issued by the Contractors State License Board, <br />and <br />
