Box 31465 <br /> Stockton, CA 95213 <br /> Re: Flag City Chevron 6421 Capitol Ave. Lodi, CA 95240 Revised SR0067880 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to install a Veeder Root 794390-407 on the underground storage <br /> tank (UST) system at the above noted address. The L4: diesel annular sensor was replaced. An <br /> addendum has been received to include the newly installed VMI 99-LD200 leak detectors for the <br /> 91 and the 87 slave turbines. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. If it is determined that <br /> a cold start is necessary, a copy of the alarm history shall be printed prior to <br /> performing the "cold start" of the Veeder Root system and made available at the time <br /> of EHD inspection. <br /> b. Leak detector test: Leak detector must be tested to confirm that the device can <br /> detect a leak of 3 gallons per hour at 10 psi. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br />
