Repeat Violations: 2, 4, 7, 14, 21, 21, 23c, 23d. <br /> #2 Secondary containment testing was last performed on 7-10-06 and was completed on 12-7-09, five <br /> months past due. Secondary containment testing is required once every 36 months. Take all necessary <br /> precautions to ensure testing is performed in a timely manner. <br /> #4 Financial responsibility documents were last submitted to the EHD for 6-25-09 and then again for <br /> 7-26-10, one month past due. Current financial responsibility documents are required to be submitted <br /> annually. Take all necessary precautions to ensure submittal of the financial responsibility documents <br /> are performed in a timely manner. <br /> #6 A copy of the UST operating permit was not found on site. A copy of the UST operating permit and <br /> all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit <br /> and retain it on site. <br /> #7 Alarm maintenance records for the following alarms were not found on site. <br /> L5 91 stp sump fuel alarm on 12-7-09, L6 87 master stp sump fuel alarm on 12-7-09, L8 diesel stp sump <br /> fuel alarm on 12-7-09, L9 91 fill fuel alarm on 12-11-09 & 12-7-09, L10 87 master fill fuel alarm on <br /> 12-12-09, L11 87 slave fill fuel alarm on 6-16-10 & 1-19-10, L13 dispenser 1/2 fuel alarm on 8-30-09, <br /> These records shall be kept on site for at least 3 years . Immediately locate and maintain the alarm <br /> maintenance records for the above listed alarms on site and submit copies to the EHD by 8-28-10. <br /> #9 UDC 9/10 is has a loose fuel filter and diesel fuel is leaking out into the UDC. The underground <br /> storage tank system shall be operated to prevent unauthorized releases. Correct immediately. Stop the <br /> leak and properly dispose of the released diesel fuel per Title 22 Hazardous Waste Regulations. <br /> #13 The 91 fill sump sensor failed to activate an audible and visual alarm when tested to ensure <br /> continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm when it detects a leak. The sensor was replaced and retested during the <br /> inspection. Obtain a permit from the EHD for this work within one business day. If a permit is not <br /> obtained within one business day, a "Failure to Obtain a Permit" penalty will apply.
