This facility was informed by the EHD to perform secondary containment testing every 36 months.On the inspection report you were reminded that this test was last performed on July 10,2006,making secondary containment testing due by July 10,2009. On August 18,2009,a reminder letter was sent by this office notifying you that secondary containment testing was still required. On September 21,2009,the EHD received a copy of secondary containment test results from Johnson Tank Fuel Systems&Testing without the required 48 hour notification.This test result indicated that this test was conducted on August 14,2009. On the test result there was a comment noted that the EHD had been provided a 48 hour notice by phone on August 12,2009.Our records do not indicate that this phone notification ever took place.Randy Johnson of Johnson Tank Fuel Systems&Testing was contacted regarding the 48 hour notice.He said he would fax documentation that he notified us. On November 9,2009,the EHD received a fax dated August 12,2009,notifying EHD of secondary containment testing for Flag City Chevron,6421 Capitol Ave.Lodi,CA on August 14,2009,in the morning by Randy Johnson. No fax was received by this office on that date,therefore the secondary containment test results of August 14,2009, will not be accepted.Correct immediately by scheduling secondary containment testing,providing the EHD 48 hours notice before conducting the required testing,and provide test results within 30 days of the completed test. Failure to correct the violations will result in continuing/additional violations. Note: All EHD staff time associated with failing to comply,including the issuance of this letter,is billed at the current hourly rate($115).
