This <br />information was required to be submitted by August 23, 2008. A Return to Compliance Letter dated April 17, 2009 <br />was previously sent requesting this facility to return to compliance. The requested information still has not been <br />received resulting in a non-compliant status for this facility. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($105).
