This information was required to be submitted by 10/26/17 . This <br /> information was not received within 30 days resulting in a non-compliant status for this facility. An operator that <br /> receives an inspection report shall have 30 days to submit a written response that includes a statement <br /> documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written <br /> response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection <br /> report. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to print L5 91 STP sump alarms from the attached alarm history on the 6/1/18 <br /> designated operator monthly inspection report. During the monthly inspection,the designated operator shall review <br /> the alarm history for the previous month, check that each alarm was documented and responded to appropriately, <br /> and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> This is a repeat violation, Class Il. <br /> 313 HSC 25290.1,25290.2,25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> The diesel product meter in UDC 9/10 had a visible seepage, indicating a leak in the primary piping. All primary <br /> containment for the UST system must be tight. Immediately have a properly licensed,trained, and certified <br /> contractor repair or replace the failed component under permit and inspection of the EHD, if required. <br /> This is a repeat violation, Class ll. <br /> 318 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87 product slave line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi.
