4 SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DEPARTMENT
600 East Main Street, Stockton, California 95202-3029
Telephone: (209)468-3420 Fax:(209)468-3433 Web:www.sigov.org/ehd/unitiii.html
CONTINUATION FORM Page: 3 of 4
OFFICIAL INSPECTION REPORT Date: 06/15/10
Facility Address: 1636 E Alpine Ave, Stockton Program: HW
SUMMARY OF VIOLATIONS
CLASS I,CLASS II,or MINOR-Notice to Comply)
Routine Hazardous Waste Generator Inspection
Notice of Violation - Class 11
#7) Failed to determine if a waste is a hazardous waste. The facility has several containers and 55
gallon drums which contain bowling ball manufacturing chemicals, lane conditioning and lane cleaning
chemicals. Some of the containers and drums appear to be old. Nelson Beaudet indicated that four
containers are waste. These containers are Aropol CF Resin (55 gallons), Component B Polyol (2 x 20
gallons) and an unlabeled 30 gallon container of oily-liquid. Make a hazardous waste determination as
specified in Title 22 California Code of Regulations on all waste in these containers and drums.
Hazardous waste can be toxic, reactive, ignitable or corrosive. Hazardous waste must be hauled from
the site by a registered hazardous waste hauler. Keep disposal records at the facility for three years.
Send a statement to the Environmental Health Department (EHD)
by July 21, 2010 which includes a list of which containers and drums are waste, the hazardous waste
determination documentation and how disposal will be handled.
Notice to Comply
#8) Managed a hazardous waste without an EPA number. The facility does not have an EPA
identification number for this address. Obtain an EPA ID number using the provided form from the
California Department of Toxic Substances Control. Use this number for disposal of hazardous waste.
Provide this EPA number to the EHD by July 21, 2010.
#11) Facility not maintained to minimize the release of a hazardous waste. A two gallon container of
lane cleaner concentrate fell over and had spilled some of its contents onto the ground. The container
was not picked up and the concentrate on the ground was not cleaned up. Cleanup all spills promptly
and make a hazardous waste determination on the spill waste as specified in Title 22 California Code of
Regulations. Correct immediately.
#14) Spill control equipment not provided. The facility does not have equipment to clean up hazardous
waste spills. The EHD recommends using rags and absorbent to cleanup spills and store the spill waste
in sealed containers or drums. Correct immediately.
#52) The facility does not have a modified contingency plan for this address posted by facility
telephones for use by employees. Fill out and post the provided form (or equivalent). Correct
immediately.
ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115).
THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
EHD Insp or: Received By: Title:
EHD 23- -003 Rev 06/01/10 CONTINUATION FORM
