The following is an itemized list of hazardous waste violations that have not been <br /> addressed for LMB INTERNATIONAL as of June 02, 2016. <br /> Open violations from March 17, 2016 inspection <br /> Violation#101 -Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, <br /> transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC web si <br /> http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. Immediately reactivate <br /> your EPA ID number and submit evidence to the EHD. <br /> Violation#117 -Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> Violation#303 -Failed to maintain all communication or alarm systems,spill control, or decontamination <br /> equipment. <br /> The fire extinguishers on site last maintenance was in May 2011. The facility did not have an eye wash station. All <br /> facility communications or alarm systems, fire protection equipment, spill control equipment, and decontamination <br /> equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of <br /> emergency. Immediately begin this practice. <br /> Violation#406 -Contaminated container not marked with date emptied or managed within one year of <br /> being emptied. <br /> A white 55 gallon empty drum on the south side of the facility did not have the date it was emptied. Containers larger <br /> than five gallons that previously held a hazardous material shall be marked with the date they were emptied and <br /> managed within one year of being emptied. Immediately mark and manage all empty containers per this section. <br /> Violation#601 -Stored hazardous waste onsite greater than 180 days. <br /> A clear 275 gallon tank of used oil on the south side of the facility was on site since 2014 according to the owner <br /> Denise Voll . Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg o <br /> waste stored on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous <br /> waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br /> Violation #605 -Failed to completely label containers or tanks of hazardous waste. <br /> A 55 gallon blue drum of liquid solvent waste on the south side of the facility was not labeled. All hazardous waste <br /> containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> - hazardous properties <br /> - physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required information. Since <br /> it cannot be determined how long the 55 gallon blue drum has been on site, immediately contact a licensed hazardOL <br /> waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br /> A clear 275 gallon tank of used oil on the south side of the facility was not properly labeled. All hazardous waste tank <br /> shall be marked with the words"Hazardous Waste" and the accumulation start date. Immediately ensure that all <br /> hazardous waste tanks are marked with the required information. <br /> Page 1 of 1 <br />
