The following is an itemized list of hazardous waste violations that have not been addressed for LMB INTERNATIONAL as of June 02, 2016. Open violations from March 17, 2016 inspection Violation#101 -Managed a hazardous waste without an identification number. This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC web si http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. Immediately reactivate your EPA ID number and submit evidence to the EHD. Violation#117 -Failed to report program data electronically. A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required fields. Violation#303 -Failed to maintain all communication or alarm systems,spill control, or decontamination equipment. The fire extinguishers on site last maintenance was in May 2011. The facility did not have an eye wash station. All facility communications or alarm systems, fire protection equipment, spill control equipment, and decontamination equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of emergency. Immediately begin this practice. Violation#406 -Contaminated container not marked with date emptied or managed within one year of being emptied. A white 55 gallon empty drum on the south side of the facility did not have the date it was emptied. Containers larger than five gallons that previously held a hazardous material shall be marked with the date they were emptied and managed within one year of being emptied. Immediately mark and manage all empty containers per this section. Violation#601 -Stored hazardous waste onsite greater than 180 days. A clear 275 gallon tank of used oil on the south side of the facility was on site since 2014 according to the owner Denise Voll . Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg o waste stored on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. Violation #605 -Failed to completely label containers or tanks of hazardous waste. A 55 gallon blue drum of liquid solvent waste on the south side of the facility was not labeled. All hazardous waste containers shall be marked with the following information: -the words"Hazardous Waste" -name and address of generator - hazardous properties - physical state -composition (contents) -accumulation start date Immediately label these containers and ensure that all containers are marked with all the required information. Since it cannot be determined how long the 55 gallon blue drum has been on site, immediately contact a licensed hazardOL waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. All hazardous waste tank shall be marked with the words"Hazardous Waste" and the accumulation start date. Immediately ensure that all hazardous waste tanks are marked with the required information.
