SANsMAQUIN <br />COUNTY <br />Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />BROOKSIDE GOLF & COUNTRY CLUB <br />3131 BROOKSIDE RD, STOCKTON <br />April 12, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />101 <br />CCR 66262.12 Managed a hazardous waste without an identification number. <br />This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, <br />transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC web <br />site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. Immediately <br />reactivate your state ID number and submit evidence to the EHD. <br />This is a repeat violation, Class II. <br />102 <br />CCR 66262.11 Failed to determine if a waste is a hazardous waste. <br />In the shop area, metal fines are generated and collected without disposing of them. The facility is not sure how to <br />dispose of the waste. Any person who generates a waste shall determine if the waste is a hazardous waste. <br />Immediately make a hazardous waste determination for the waste and manage it according to Title 22 hazardous <br />waste regulations. Particles 100 microns or smaller must be handled as hazardous waste if the metal is determined <br />to be a hazardous waste. Submit a statement and supporting documentation explaining how the metal fines are <br />going to be managed. <br />The following items were observed on the south side of the yard area: <br />-Eight rusted one pint paint cans <br />-One rusted 55 gallon steel drum containing rainwater, absorbent, and other material <br />-Three blue 55 gallon poly drums containing an unknown material <br />-Two blue and four white 30 gallon poly drums containing an unknown material <br />-One white 16 gallon poly drum containing an unknown material <br />-Five 5 gallon plastic buckets containing an unknown material <br />Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately make a <br />hazardous waste determination for the items listed above and manage them according the Title 22 hazardous waste <br />regulations. Submit a statement and supporting documentation explaining how each item was managed. <br />This is a minor violation. <br />104 <br />CCR 66262.34(d)(2) No modified contingency plan. <br />The contingency plan was out of date. The plan listed Ryan Cheers as the emergency coordinator instead of the <br />current one, Steve Showers. There must be at least one emergency coordinator on site or on call to coordinate <br />emergency response measures, and the following information must be posted by a phone: the name and phone <br />number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire <br />alarm; and the phone number of the fire department, unless the facility has a direct alarm. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />FA0010096 PR0514166 SC001 04/12/2019 <br />EHD 22-01 Rev. 09/27/2018 Page 4 of 7 Small Quantity Hazardous Waste Generator OIR <br />1868 E.
