Complaint Investigation Rep <br />Tri -Valley Recycling <br />Photo13: Container belonging to Alco Iron & Metal Company with phone numbers in <br />San Leandro and Vallejo <br />After the walk through inspection, we came back to the tri -Valley Recycling office area. <br />I asked for the documents related to the shipment of CRT's that were shipped out by a <br />truck belonging to "Diaz Trucking". Tri -Valley Recycling submitted copies of flyers, <br />records of estimated weights of CRTs during pick up and the names and addresses of <br />residents who gave e -waste to the facility. Tri -Valley recycling made copies of driver's <br />licenses of customers who dropped off e -waste at the facility. Ms. Cravens, the Office <br />Manager told me to come back on the following day to review documents relating to e - <br />waste recycling. She said she did not have e -waste recycling documents, and the <br />owner of the company will provide the documents to me on the following day. <br />I drove back to the facility on the following day, May 20, 2011, and Mr.Owens, the <br />Logistic Manager, greeted me at the front desk. He gave me a copy of the 2010 annual <br />report. He said he could not locate the original recycler/ collector notification to DTSC. <br />I gave him the SOO and the SOV and explained each violation. I asked him to dispose <br />the stored batteries in the facility. I gave him a copy of a void manifest and explained <br />how a manifest should be filled out by the generator, transporter and the treatment <br />storage and disposal facility. I asked him to the mail generator copy of the manifest to <br />DTSC. I asked him to obtain an EPA ID number and use a manifest to dispose the <br />batteries. I said he must employ a hazardous waste transporter with a valid EPA ID <br />number and transport the batteries to a hazardous waste disposal site that could <br />receive batteries. Mr. Owens said he will complete the corrective actions and submit all <br />required documents on time to DTSC. <br />16 of 20 <br />
