Please fill out the attached form and return. Our accounting department will process and mail an invoice, once paid they will send out the permit. Please include a Tax ID number if available. I will fill out the form listed on the bottom that also needs to be submitted. The area inspector is Muniappa Naidu (209) 468-3439. Please contact me if you have any questions. Amy gave me your EPA ID number for this facility: CAL000352796 and stated that you are in the process of getting it reactivated.
