Within 30 days of one the following events,business shall electronically update the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> A site map was missing the entrances and exits, the emergency response equipment, the evacuation staging area, <br /> the loading area,storm and sewer drains, and emergency shut-offs. A site map shall contain a north orientation, <br /> loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a <br /> site map element is not applicable for your facility then list it on the map and label as"NA". This was corrected on site <br /> This is a minor violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Facility failed to retain records of training for employees handling hazardous materials. The business plan shall <br /> include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training <br /> shall be documented electronically or by hard copy and shall be made available for a minimum of three years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification Form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 5/15/19. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate$152.
