SAN J O A Q U I N Environmental Health Department <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree#03771 7528 Pacific Ave, Stockton Aril 23, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> The following observations were made in the hazardous waste storage area at the time of inspection(see photo IMG <br /> 5497): <br /> -A State-Regulated, Toxics, Electronics, OTC Pharmaceuticals, Universal Waste(STOU)HW tote was found lacking <br /> an accumulation start date and did not have the contents marked off on the HW label(IMG 5502).The STOU HW tote <br /> contained the following items(IMG 5503): <br /> -Love My Carpet carpet deodorizer <br /> -Nack-A-Pack Surprise Greetings <br /> -Nature's Measure Hair, Skins& Nails Tablets <br /> -a green container containing unknown pills <br /> Initially, Albert marked the contents off on the label, but did not mark an accumulation start date on the HW label until <br /> the inspector pointed out that the accumulation start date was still lacking.Albert marked the accumulation start date <br /> on the label based on the earliest date found written on an item that was in the tote. <br /> The following was observed near the front of the facility: <br /> -A leaking Bolero Peonies&Pear Body Oil was found leaking in a HW bag in an open Dollar Tree basket labeled as <br /> "Damaged Goods"(IMG 5530, 5532-4). The hand-held scanner indicated the item was not hazardous(IMG 5536), <br /> however Albert stated, although he did not know who placed the item in the HW bag, he would have also classified the <br /> item as HW.Albert placed the item into an STOU HW tote in the HW storage area. <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition(contents) <br /> -accumulation start date <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection.
