SANJ O A Q, ` I N Environmental Health Department <br /> - C i_ U '11 rr , <br /> WASTE DISCHARGE/SPILL RESPONSE <br /> NOTICE TO ABATE <br /> Responsible Party: PACIFIC GAS AND ELECTRIC COMPANY Date: Jun 06, 2018 <br /> RP Mailing Address: 1108 MURPHY GRADE RD Record ID#: C00046832 <br /> ANGELS CAMP, CA 95206- Program Element 2546 <br /> Spill Location: 6440 AVIATION DR, STOCKTON Release/Spill Response (excluding Joint Team) <br /> NATURE OF WASTE DISCHARGE/WASTE RELEASE: <br /> Six gallons of transformer oil was released from a pad mounted transformer caused by a vehicular impact. <br /> OBSERVATIONS: <br /> Met with Dave Larson from PG&E. The spill had been cleaned up. Approximately 6 gallons of oil spilled. The accident was <br /> caused by a farmer driving equipment. Four 55 gallon drums of waste was generated. The waste is currently labeled as <br /> hazardous waste with a pending analysis. Samples are going to be run for PCBs. <br /> VIOLATIONS: <br /> • Discharge of any waste, including sewage <br /> (CA Health and Safety Code 5411) <br /> El Discharge of waste or sewage to water or within 150 ft. of waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br /> • Improper disposal of a hazardous waste <br /> (CA Health and Safety Code 25189.5) <br /> Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> Other Violation <br /> CORRECTIVE ACTIONS /ORDER: <br /> • Investigate and abate contamination under the oversite of the appropriate lead agency. <br /> Immediately. <br /> • Submit copies of hazardous waste manifests/disposal records to the EHD. <br /> Submit copies of disposal record and lab analyticals to EHD. <br /> ° Other Correction <br /> COST RECOVERY: All EHD time associated with this Spill Response is billed to the responsible party at the current hourly <br /> rate.
