The following is an itemized list of hazardous waste violations that have not been <br /> addressed for JAAS AUTO SERVICE as of Apri129, 2019. <br /> Open violations from November 30, 2018 inspection <br /> Violation #608 -Failed to properly manage used oil and fuel filters. <br /> Bills of lading for the recycling of used oil filters for the last three years were not found on site. A copy of each bill of <br /> lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters <br /> for the last three years, maintain them on site, and submit copies to the EHD. <br /> In the used oil storage area: <br /> -One black 55 gallon metal drum containing metal filters (full)was observed without an accumulation start date. <br /> -Six 5 gallon plastic buckets containing metal filters(all full)were observed without a label. The containers were <br /> open without a lid. <br /> -Two 3 gallon plastic buckets containing metal filters (all full)were observed without a label. The containers were <br /> open without a lid. <br /> -One black 5 gallon drain pan containing metal filters (full)was observed without a label. The container was open <br /> without a lid. <br /> Used oil filters shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as"drained used <br /> oil filters," and marked with an accumulation start date. Immediately properly containerize and label all drained used <br /> oil filters. Ensure that all used oil filters are managed in this way. <br /> All off the drained metal oil filter containers listed above did not have an accumulation start date. The latest record <br /> on site for the disposal of the oil filters was 8/9/2017. Facilities that generate up to one ton of used oil filters per <br /> year may store the filters up to one year. Facilities that generate one ton or more of used oil filters may store the <br /> filters for up to 180 days. Immediately recycle these filters under a bill of lading and submit a copy of the bill of <br /> lading to the EHD or send in records showing that the filters have not been on site for more than 180 days. <br /> Open violations from August 02, 2016 inspection <br /> Violation #101 -Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number (CAL000312657) is inactive. A hazardous waste generator shall not <br /> treat, store, dispose of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. <br /> Go to the DTSC web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID <br /> number. Immediately reactivate your state ID number and submit evidence to the EHD. <br /> Violation#103 - Failed to retain hazardous waste determination including waste analysis on site for 3 years. <br /> No waste determination analysis found on site. Operator must keep a waste determination including waste analysis <br /> on site for 3 years. Provide proof the EHD that the waste analysis and determination is stored on site. <br /> Violation #107 - Failed to prepare and implement a contingency plan. <br /> The facility failed to prepare a contingency plan. The owner or operator shall prepare a contingency plan designed <br /> to minimize hazards to human health or the environment from fires, explosions, or any unplanned release of <br /> hazardous waste to air, soil, or surface water and immediately implement that plan whenever there is a fire, <br /> explosion, or release of hazardous waste which could threaten human health or the environment. Immediately <br /> prepare a contingency plan and submit a copy to the EHD. <br /> Violation #114- Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for the last three years were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility on site for three <br /> years and have them readily available for review. Immediately locate a copy of all manifests for the last three years, <br /> maintain them on site, and submit copies to the EHD. <br /> Page 5 of 17 <br />
