Hwy 88 Lockeford, CA 95237 4-14-11 revised SR0061453 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the 4-14-11 revised application for a permit to include installation of OPW 10-series shear <br /> valves and bravo MO-400 offsets between dispenser and piping in the under dispenser <br /> containment. Original application includes install of two Gilbarco advantage (3+1) Dispensers <br /> and two Gilbarco Advantage (3+0) Dispensers, four Bravo conversion frames, install a Red <br /> Jacket FX2DV on the diesel tank, de-manifold vent risers, install OPW 23-033 vent cap on new <br /> diesel vent riser, install husky 5885 pressure/vacuum vent cap on remaining gasoline vent <br /> risers, convert the existing Premium (91) UST to contain Diesel, and convert the existing <br /> Midgrade (89) UST to Premium (91) at the above noted address. The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Leak detector test: Diesel and 91 leak detectors must be tested to confirm that the <br /> device can detect a leak of 3 gallons per hour at 10 psi. <br /> b. Secondary containment testing on the diesel and 91 UST's. <br /> c. Line integrity test on the diesel, 91, and 87 product lines are required. <br /> d. Verify vapor recovery line and spill container are capped off prior to backfill. <br /> e. Verify Bravo conversion frame is installed per manufacturer's instructions. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br />
