Lodi, CA Program: UST <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> On site to conduct the routine inspection and witness the monitoring system certification. <br /> #3 1 did not observe the current tank forms (13 Forms) in the facility file. The current tank forms (Form B) <br /> must be submitted to this office. Correct immediately. Submit the current tank forms (Form 13) for this <br /> facility to this office by 6-24-10. <br /> #5 1 did not observe the current monitoring and response plans (D & E Forms) in the facility file. The <br /> current monitoring and response plans (D & E Forms) must be submitted to this office. Correct <br /> immediately. Submit the current monitoring and response plans (D & E Forms) for this facility to this <br /> office by 6-24-10. <br /> #12 The 89 Vaporless Manufacturing Inc. (VMI) mechanical line leak detector failed the 3 gal/hr at 10psi <br /> test. The technician installed a new VMI mechanical line leak detector 99-LD2000 serial number <br /> 04041138 and tested it to pass in the presence of the inspector. Corrected on site. Immediately submit a <br /> permit application for the replaced leak detector. <br /> #13 The under dispenser containment (udc) 5/6 89 & 91 and udc 3/ 89 float and chain leak detectors did <br /> not trip the shear valve when tested with water. The technician adjusted the float and chain leak <br /> detectors to trip the shear valve when tested with water in the presence of the inspector. Corrected on <br /> site. <br /> #18 The 91 spill container failed the 1 hour hydrostatic test. Immediately discontinue deposition of fuel <br /> into the 91 tank until the 91 spill container has been repaired/replaced by a licensed, trained, certified <br /> contractor under permit from San Joaquin County Environmental Health Dept. Correct immediately. <br /> #19 This site uses ball floats set at 90% and fill sumps for overfill prevention. I did not observe ball floats <br /> in any of the vapor risers in the fill sumps, the west 89 submersible turbine pump (stp) sump riser, nor in <br /> the west 87 stp sump riser. Immediately discontinue deposition of fuel into the 91, 89, and 87 tanks until <br /> an overfill prevention mechanism per title 23 underground storage tank regulations has been installed <br /> by a licensed, trained, certified contractor under permit from San Joaquin County Environmental Health <br /> Department. Correct immediately. <br /> Note: The 91 stp sump sensor was rewired so that the splice would be in the j-box. This sensor was <br /> rewired and tested to pass in the presence of the inspector. Corrected on site. Immediately submit a <br /> permit application for the rewired sensor. <br /> Complete and submit the return to compliance certification, corrective action statements, and any <br /> supporting documentation to this office by 6-24-10. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector Aris Cacapit j Receiv dZy: Title: <br />
