The following is an itemized list of hazardous materials violations that have not been
<br /> addressed for Aramark Uniform & Career Apparel, LLC-Stockton as of May 14,
<br /> 2019.
<br /> Open violations from April 04, 2019 inspection
<br /> Violation #5 -Failed to complete and/or submit hazardous materials inventory for all reportable materials on
<br /> site.
<br /> 219 c.f. of acetylene, 312 c.f. of compressed oxygen, at least 1,000 gallons of polyclear CD-580 4H, 9,000 pounds
<br /> of polyclear 390 (solid), 1,000 gallons of polyclear 390 (liquid), and at least 120 gallons of hydraulic fluid inside six
<br /> Ellis 900 washing machines were found on site that have not been reported. Any material that meets or exceeds
<br /> the reportable quantity shall be reported in the facility's business plan. Immediately log into the California
<br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material
<br /> inventory, and submit to the EHD for approval.
<br /> The Hazardous Materials Inventory forms for emax alkali, performance detergent, bleach, turbo fresh, advacare,
<br /> performance booster, injection sour, ecolab fabric relaxant, trubo tex, tex stat II, turbo crisp, polyclear 1267,
<br /> boilercare 1011, nalco 359, and nexguard 22310 are not compete.
<br /> • The hazardous component information for emax alkali, performance detergent, bleach, turbo fresh, advacare,
<br /> performance booster, injection sour, ecolab fabric relaxant, trubo tex, tex stat II, turbo crisp, polyclear 1267,
<br /> boilercare 1011, nalco 359, and nexguard 22310 has not been completed.
<br /> • The amounts of liquid emax alkali, performance detergent, bleach, turbo fresh, advacare, performance booster,
<br /> injection sour, ecolab fabric relaxant, trubo tex, tex stat ll, turbo crisp, and polyclear 1267 must be identified in
<br /> gallons vs. pounds.
<br /> • The storage pressure for performance booster has not been completed; the storage pressure and storage
<br /> temperature for injection sour and polyclear 1267 have not been completed,
<br /> • The 1,000 gallon tank of sodium hydroxide liquid has been identified as sodium hydroxide"solid".
<br /> This information must be current and submitted as part of the business plan. Immediately log into the California
<br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and
<br /> submit to the EHD for approval.
<br /> Violation #6 - Failed to complete and/or submit a site map with all required content.
<br /> The map does not show loading areas, adjacent streets, storm and sewer drains, access and exit points,
<br /> emergency shutoffs, evacuation staging areas, the hazardous materials handling and storage areas (compressed
<br /> gases and outside waste storage on the north and west sides of the facility), and a legend that defines all of the
<br /> codes/numbers used on the map. A site map shall contain a north orientation, loading areas, internal roads,
<br /> adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas,
<br /> hazardous material handling and storage areas, and emergency response equipment. If a site map element is not
<br /> applicable for your facility then list it on the map and label as"NA". Immediately log into the California
<br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information,
<br /> and submit to the EHD for approval.
<br /> Violation #8 - Failed to submit a training program or program is not reasonable or adequate for facility.
<br /> The training program lacks training of the immediate notification contacts to the appropriate local emergency repose
<br /> personnel and the Environmental Health Department. Training programs for all employees and annual training,
<br /> including refresher courses, shall include safety procedures for the event of a release or threatened release of a
<br /> hazardous material, including, but not limited to, familiarity with the facility's emergency response plans and
<br /> procedures. These training programs may take into consideration of each employee. Immediately prepare or
<br /> update the facility's training program, log into the California Environmental Reporting System (CERS) at
<br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
<br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to
<br /> date.
<br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting
<br /> System (CERS) annually by January 15 for 2018 and 2109. The hazardous materials inventory shall be submitted
<br /> by January 15 of each calendar year and may be submitted beginning November 1 of the previous year.
<br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to
<br /> the EHD for approval.
