The primary and secondary contact information <br /> has not been updated. Please update the name and phone number for the contacts information. <br /> This information must be current and submitted as part of the business plan. This violation has been corrected on <br /> site. <br /> This is a repeat violation, Class II. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> The Hazardous Materials Inventory forms for diesel were not complete. Obsolete federal hazard classes were listed <br /> on the chemical form. Please update the federal hazard categories using an updated Safety Data Sheet(SDS). <br /> This information must be current and submitted as part of the business plan. This violation has been corrected on <br /> site. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. Line C3 was missing the local Unified Program Agency <br /> (UPA)witch is 209-469-3420. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c)Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request.
