Any material that meets or <br /> exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/,add the materials to the hazardous material <br /> inventory,and submit to the EHD for approval. <br /> This was corrected on site. <br /> This Ise minor violation. <br /> 8 HSC 25505(ax2)Failed to complete and/or submit a site map with all required content. <br /> The submitted facility site map did not meet the requirements.The facility site map did not have north orientation, <br /> loading areas,internal roads storm and sewer drains, access and exit points,emergency shutoffs, evacuation <br /> staging areas,hazardous material handling and storage areas,and emergency response equipment. A site map <br /> shall contain a north orientation,loading areas, internal roads,adjacent streets, storm and sewer drains, access and <br /> exit points,emergency shutoffs,evacuation staging areas, hazardous material handling and storage areas, and <br /> emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as'NA". Immediately log into the California Environmental Reporting System(CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This Ise minor violation. <br /> 9 HSC 2550%ax1)Failed to annually review and certify that the business plan is complete,accurate,and up to date. <br /> The business plan information was not reviewed and resubmitted in the California Environmental Reporting System <br /> (CERS)within the required time period.The last submittal was on 3/22/2019 and the submittal before that was on <br /> 5/3/2017. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be <br /> submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, <br /> enter the correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork by May 19, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate($152).
