FACILITY THREAT PLAN : HAZELTON <br /> This document works in conjunction with Public Health Services Department Emergency Plan. Although both <br /> address emergency responses and responsibilities,the Facility Threat Plan is facility specific,detailing items that <br /> will only affect particular buildings of PHS, i.e., evacuation routes and fire extinguisher placement. If the <br /> directives are not in Facility Threat Plan,they are detailed in the Department Emergency Plan. <br /> FFACILITY THREAT COORDINATOR <br /> The FTC is responsible for evaluating threats and making protective action decisions with public safety agencies. <br /> During active threats,the FTC supervises implementation of a protective action by the department,disseminates <br /> information to Area Wardens regarding the event,and maintains the Facility Threat Plan. <br /> AUTHORITIESALERTING <br /> When an employee is witness to an emergency situation/threat(fire,toxic exposure, medical situation,etc.), <br /> expectation is that he/she will contact the nearest supervisor and/or call 9-9ii(from County phones)or 911 from <br /> cell phones. The next person to contact is the Facility Threat Coordinator. <br /> RESPONSEINITIAL <br /> Once a threat occurs,the Facility Threat Coordinator and Alerting Aides will immediately report to the Internal <br /> Command Post. The FTC will do take the following steps: <br /> i. Activate the Internal Command Post <br /> i. Develop a message to Area Wardens that states <br /> 3. the problem <br /> 4. action to be taken <br /> 5. Internal Command Post is activated <br /> 6. Method for contacting him/her <br /> 7. Area Wardens are to check in via i-com radio <br /> 8. Direct Alerting Aides to call each Area Warden,starting with primary assignee and provide initial <br /> message from FTC. <br /> COMMUNICATIONS <br /> NOTIFICATION OF WARDENS <br /> The FTC or Alerting Aides are responsible for making personal contact with all of the Area Wardens. <br /> Area Wardens will then access and set their assigned i-com radio to the following settings: <br /> Radio Channel: 7 <br /> Group Code: of <br /> Smart Ring(SR): OFF <br /> They will then check in with the FTC via i-com radio. If the radio check does not work,Area Wardens are to call <br /> the Internal Command Post at 209-468-3409. <br /> 1 SJCPHS Facility Threat Plan: Hazelton <br />
