Secondary containment testing was performed on August 25, 2011; the test report was submitted <br /> to the EHD on October 21, 2011. A copy of the test report must be submitted within 30 days of the test. <br /> Ensure that all future test reports are submitted to the EHD within 30 days of testing. <br /> 206. The designated operator employee training for Cynthia Perez was not performed. According to Ms. <br /> Perez, she received the appropriate training approximately two weeks ago, but the training record could <br /> not be located. The designated operator shall train facility employees for which he or she is responsible <br /> in the proper operation and maintenance of the UST system once every 12 months. A trained employee <br /> shall be on site at all times. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD by June 29, 2012. <br /> 305. The Bravo box float and chain in the UDC 7/8 (diesel) sump failed to stop the flow of product at the <br /> dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br /> alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> adjusted the float and verified functionality.
