Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on May 29, 2012 and a test report was submitted to the EHD on October 1, 2012. A copy of <br /> the test report must be submitted within 30 days of the tests. Ensure that all future test reports are <br /> submitted within the required timeframe. <br /> 204. The designated operator failed to check and note the correct test dates for the monitoring system <br /> certification, spill container testing, and secondary containment testing on the June 2012 and July 2012 <br /> designated operator monthly inspection reports. During the monthly inspection, the designated operator <br /> shall check that all required testing and maintenance for the UST system have been completed and <br /> document the dates they were done. Ensure that designated operators performing monthly inspections <br /> at this facility are including all of the required information on the reports. <br /> 310. Liquid was observed in the diesel STP sump. If water could enter into the secondary containment <br /> by precipitation or infiltration, it must be removed and disposed of properly. The liquid was removed by <br /> the technician at the time of inspection.
