Any material that <br /> meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous <br /> material inventory, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show four(4)storm drains located on the property, carbon dioxide gas handling and storage <br /> areas, and some of the codes/symbols used in the map are not included in the legends. A site map shall contain a <br /> north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS) annually by January 15 for 2018 and there has been no submittal for 2019. The hazardous <br /> materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning <br /> November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or <br /> updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection, three 50 pound cylinders of carbon dioxide gas were found on site that have not been <br /> reported and the updated information has not been submitted.
