Linne Rd <br /> Tracy, CA 95378 <br /> Correspondence recently received from Polly Lowry of the California Regional Water Quality Control <br /> Board, Central Valley Region (CVRWQCB), indicates a conditional approval to a report submitted by your <br /> firm for additional subsurface investigation and monitoring well installation at your site. San Joaquin <br /> County Public Health Services, Environmental Health Division (PHS/EHD) requires a permit for this <br /> activity and an application fee of$89 to be submitted to our office prior to commencement of the work. <br /> Although PHS/EHD is not the lead agency for oversite for your site, we do require copies of all work <br /> plans, reports, and correspondence be on file with our office prior to the commencement of any <br /> investigation within our county. Please submit to this office the report entitled "Work Plan for Northern <br /> Extent Investigation and Remedial Action Alternatives" along with all future correspondence. <br /> In addition, a prepayment for services is required whenever there is a change in investigative phases or a <br /> change in billing/responsible party information.
