12.1 Fire and Fire Extinguishers <br /> Type ABC fire extinguishers will be available on-site to contain and extinguish small fires. The <br /> local fire department should be summoned (911) in the event of any fire on site. <br /> 12.2 Communication (Telephones) <br /> A Cellular phone may be available at the site. If a cellular phone is not available, then the SSO <br /> will locate the nearest telephone prior to the start of work,mark it on the site map, and inform <br /> site personnel of its location. A list of emergency numbers will be prominently posted at this <br /> telephone. <br /> 12.3 Incident Report <br /> In the event of an injury or illness, work will be stopped until the SSO and the RHSM have <br /> determined the cause of the incident and have taken the appropriate action. Injury or illness, <br /> regardless of severity,will be reported in the form of an accident report. <br /> 12.4 Operation Shutdown ` <br /> Under certain extreme hazardous situations the on-site geologist, SSO, or;subcontractor's safety <br /> representative may request that drilling or sampling procedures be temporarily suspended while <br /> the underlying hazard is corrected or controlled. The SSO will have ultimate authority for <br /> operations shutdown and restart. <br /> A-9 <br />
