Box 388 • Stockton, CA 95201-0388 ca�raN`° <br /> 2091468-3420 <br /> PETER WALLACE MAILED MAR 13 1996 <br /> P O BOX 99308 <br /> STOCKTON CA 95201 <br /> RE: HESS-DUBOIS CLEANERS (former) SITE CODE: 2504 <br /> 348 West Harding Way <br /> Stockton, CA 95204 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS- <br /> EHD) has reviewed the combined second and fourth quarterly reports as submitted <br /> by your consultant, Advanced GeoEnvironmental and has the following comments. <br /> No mention is made to the missing third quarter (July, August, and September) 1995 <br /> and PHS-EHD hereby directs you to comply with the following: <br /> Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (California <br /> Code of Regulations, Title 23, Subchapter 16) a quarterly monitoring report shall be <br /> submitted to the Environmental Health Division (PHS-EHD) and the Central Valley <br /> Regional Water Quality Control Board (CVRWQCB) for every quarter until the <br /> investigation and cleanup of the site is deemed adequate. The report is to show the: <br /> status of the investigation and cleanup activities including the results of <br /> all investigations completed to date; <br /> - proposed, continuing, or next phase of investigation; <br /> method of cleanup proposed or implemented and schedules for the <br /> completion of the investigation of the site and remediation; <br /> - method and location of disposal of the released hazardous substance <br /> and any contaminated soil or groundwater; <br /> manifest required for transport of hazardous substances; <br /> w monitoring well data including groundwater elevation, pH, conductivity, <br /> temperature, gradient, and status of wells; <br /> analytical results of groundwater or soil sampling; and <br /> site map showing the "zero-line" of contamination, if known, and <br /> changes in the above measurements over the last quarter. <br /> Every quarter a report is due that shows, at the very least, the above mentioned <br /> items. <br /> A Di%l ion of Spin Joaquin Counts•Heaith Care Services <br />
