On page 6 of the application for Priority Class C —Other Business, you <br /> marked the box indicating that the business is dominant statewide in its field of operation. <br /> In order to qualify for this priority,-the answ-er must be no, this business is not dominant <br /> statewide in its field of operation. <br /> 8. Claimant is required to have current financial responsibility documents on file. You did <br /> not submit the Certification of Financial Responsibility. You submitted a Letter from <br /> Chief Financial Officer that is not correct. If you provide a copy of the pre 1990 permit, <br /> you must only demonstrate $5,000 of annual aggregate. If you did not permit the tanks <br /> between 1984 and 1990, you must demonstrate $10,000 of annual aggregate. Enclosed is <br /> a Financial Responsibility Guide that explains the requirements. The Certification form <br /> is tabbed for your convenience and must be completed along with the applicable <br /> supporting documents. Please submit the original documents to the local regulatory <br /> agency and forward a copy to us.
