Please submit the destruction permit application and the $60 application fee along with a brief workplan
describing the method and materials proposed for the destructions. Please remove the 55 gallon barrels currently
stored on site and the existing stockpiled clean soils on site may be spread over the site.
With the destruction of all of the monitoring wells, and the removal of the drums, PHS-EHD will consider the
site investigation and remedial action for the underground storage tank(s) as completed and this site certified.
Thank you for your cooperation throughout this investigation.
With the provision that the information provided to this agency was accurate and representative of site
conditions, no further action related to the underground tank release is required based on the available
information set forth in California Code of Regulations, Title 23, Division 3, Chapter 16, Article 11,
Section 2721(e) (Underground Storage Tank Corrective Action Regulations).
Ernest M.
