HARDING WAY <br /> STOCKTON, CA 95202 <br /> On May 18, 1994 the Technical Review Committee (TRC) of the San <br /> Joaquin County Public Health Services, Environmental Health <br /> Division (PHS-EHD) met to listen to evidence in support of site <br /> closure as presented by your consultant, Geological Audit <br /> Services. The TRC has made a decision and recommends the <br /> following in order to evaluate your site for certification. <br /> Due to substantial groundwater contamination noted on September <br /> 13, 1993, the TRC has determined that this site now qualifies as <br /> a "groundwater" site and as such, concurrence from the Central <br /> Valley Regional Board, as well as PHS-EHD, is required for <br /> closure. <br /> Enclosed please find a copy of the Central Valley Regional Board <br /> Requests for Site Closure, Addendum to Appendix A. This document <br /> shall be used as the guideline for preparation of the Closure <br /> Summary Report that shall be submitted to PHS-EHD. Upon approval <br /> of this report, PHS-EHD will forward its recommendation to the <br /> Board for concurrence. The Board has thirty (30) days to comment <br /> on the recommendation or the report is automatically approved. <br /> In the interim, PHS-EHD hereby modifies the sampling and <br /> monitoring events as previously directed. Both sampling and <br /> monitoring of the onsite and offsite wells shall be conducted on <br /> a quarterly basis beginning this second quarter (May, June, and <br /> July) . Groundwater sampling shall follow previously directed <br /> protocol and shall be sampled under PHS-EHD witness by <br /> appointment. A quarterly monitoring report shall be submitted to <br /> PHS-EHD by the 15th of the month following the quarter's end. <br /> PHS-EHD would like to thank you for your cooperation and patience <br /> throughout this investigation and associated site activities. It <br /> is the recommendation of the TRC that no further subsurface <br /> investigation be required at this time.
