Madison <br /> San Joaquin County Public Health Services Environmental Health Division (PHS/El-ID) has reviewed the February 28 <br /> First Quarter 1994 Monitoring Report prepared by RESNA for the above referenced sites. <br /> In a November 16, 1993 letter FHS/EHD stated that a workplan to further investigate the site be submitted by January <br /> 15, 1994 in addition to the quarterly monitoring report. In a February 2, 1994 letter RESNA stated that a workplan is <br /> currently being prepared to further investigate and remediate the site.A workplan was not included as part of the fust <br /> quarter monitoring report . <br /> In order to expedite the investigation process a permit application and fee of$89 should be submitted with the workplan. <br /> If the workplan is not submitted by May 1, 1994,PHS/EHD may complete the referral process at which time the Central <br /> Valley Regional Water Quality Control Board may initiate enforcement procedures. In addition,if you do not comply with <br /> PHS/EHD directives you may jeopardize your eligibility for reimbursement from the State Water Resources Control Board <br /> Cleanup Fund Program. <br /> To date PHS/EHD has not received a report on the free product removed from MW-3 on February 2,1994.
