INTRODUCTION <br /> At the request of Mr Jeffery Setness of Jeff B Setness Professional Law Corp , Advanced <br /> ' <br /> Geo Environmental, Inc (AGE) has prepared this quarterly report summarizing field activities <br /> performed during the second quarter of 2000 at 423 North Madison Street,Stockton, California The <br /> scope of work performed on the site for the quarter included monitoring concentrations of dissolved <br /> petroleum hydrocarbons in ground water on the site, Installation of four soil borings, and preparation <br /> of this report The site setting is Illustrated in Figure 1 Structures and monitoring well locations are <br /> illustrated in Figure 2 <br /> ' f Site background information was summarized in Revised Corrective Action Plan -February 1997, <br /> Site Investigation Report- UST No 8,,dated 10 February 1998 and Quarterly Report-August 1998, <br /> ' prepared by AGE <br /> ' 2.0. PROCEDURES <br /> On 02 May 2000, four soil probe borings (15B-6 through 1513-9)were advanced at the site to further <br /> ' assess the extent of contaminants and investigate geophysical anomalies identified during previous <br /> assessment investigations at the referenced site The field work was conducted as outlined in the <br /> PHS-EHD-approved Site Investigation Work Plan -April 2000 dated 25 April 2000 <br /> On 09 May 2000, ground water monitoring and sampling was performed at the site on wells MW-1, <br /> MW-2,MW-4, MW-5,MW-8,MW-11, MW-12 MW-13, MW-18, MW-19A MW-19B, MW-20A, <br /> MW-20B, MW-21A and MW-21B in accordance with the revised monitoring schedule established <br /> at a 03 April 1998 meeting with the PHS-EHD <br /> ' Field activities were performed in accordance with guidelines for investigation of underground <br /> storage tank (UST) sites established by the Regional Water Quality Control Board- Central Valley <br /> Region (RWQCB-CVR) and by the San Joaquin County Public Health Services - Environmental <br /> Health Division (PHS-EHD) <br /> i <br /> 21 PROBE BORING ACTIVITIES <br /> Soil probe bonngsl5B-6 through 1513-9 were advanced utilizing a van-mounted Geoprobe 5400 <br /> equipped with 1 125-inch diameter probe rods with an attached 2 15-inch diameter soil sampler, <br /> using "direct push" technology Soil probe borings 1513-6, 1513-7 and 15B-9 were advanced to a <br /> depth of 15 feet below surface grade (bsg) Soil probe boring 1513-8 was advanced to a depth of 20 <br /> • Advanced GeoEnvlronmental,Inc S <br /> 1 <br />
