CITY OF LODI ENCROACHMENT PERMIT CONDITIONS <br /> ATTACHMENT TO ENCROACHMENT PERMIT NO 2003-095 <br /> NOTIFY CITY INSPECTOR AT START OF WORK <br /> 1 Affected portions of public sidewalk to be closed in vicinity of work in progress, proper signage <br /> required See Note No 11, "Sidewalk Closed Ahead" signs required at nearest crosswalks, <br /> 2 Barricades required at work site No open excavations except at time of work, <br /> 3 Backfill per City of Lodi Standard Plan No 506, match finish grades, all work shall comply with <br /> City of Lodi Standard Plans and Construction Specifications <br /> 4 No foreign material to enter City storm drain system, no flushing, no sweeping into gutter <br /> Accumulation of sand or other material, which might interfere with drainage, constitute a <br /> hazard to traffic and prior to a pending storm, will not be permitted and must be removed <br /> 5 Waste water generated by concrete work to be captured and vacuumed prior to entering city <br /> storm drain system, <br /> $ No storage of materials on sidewalk or street Use of private property shall be only at <br /> permission of property owner, <br /> PROPER NOTIFICATION of proprietors/residents required, access to driveways to be <br /> maintained Access to public facilities not to be obstructed, <br /> 8 Backfill shall be per approved work plan <br /> Pavement. Drill pavement Backfill with grout slurry to existing 50% of A B , deep lift asphalt <br /> concrete to existing finish grade Temporary cold patch acceptable with hot mix to follow <br /> 9 A locking waterproof cap is to be placed on top of sealed well casing to prevent unintentional <br /> introduction of materials into well, <br /> 10 Turfed, landscaped areas and sprinkler systems disturbed shall be returned to a like or better <br /> condition, <br /> 11 NOTIFY MUNICIPAL SERVICE CENTER, Rich Lenfestey, 333-6740, regarding damaged or <br /> destroyed public water, sewer, storm services and mains, facilities and structures, <br /> 12 Public streets to remain open at all times Precautionary measures shall be taken with <br /> construction vehicles and equipment that may create sight-obstruction for traveling public <br /> Extraordinary measures in traffic control to be exercised in proximities of street intersections <br /> and those with high traffic volumes One-way traffic requires flaggers present at all times, <br />
