Ms. Kristin Shelton
When present, Ground Zero personnel removed free product by bailing from wells MW6R and
' MW9R. The removed material was transferred into a labeled DOT-approved drum and stored on
site according to 49 CFR part 172 and 40 CFR part 262.
' Approximately 0.5 gallon of free product/water mixture was removed from well MW6R by
bailing during 2015. The total quantity of free product/water removed from MW6R since
October 2007 is approximately 131 gallons. No free product/water mixture was removed from
' well MW8 during 2015. The total quantity of free product/water removed from well MW8 since
April 1994 is approximately 946 gallons. Approximately 8 gallons of free product/water mixture
were removed from well MW9R during 2015. The total quantity of free product/water removed
' from MW9/9R since October 2003 is approximately 524 gallons. During 2015 well VEWI-D
was utilized for vapor extraction and was not monitored for free product. The total quantity of
free product/water removed by bailing from VEWI-D since June 2010 is approximately 125
gallons. Product thickness and recovery data for wells MW6R,MW8, MW9R and VEWI-D are
summarized in Tables 3, 4, 5 and 6, respectively, which are included in Attachment B.
' Groundwater Sampling
' Site wells were sampled in accordance with the monitoring and sampling schedule set forth in
MRP No. R5-2002-0827.
Waste Disposition
On April 29, 2015, 420 gallons of product/water mixture and purge water was transported by
' American Valley Waste Oil, Inc. to the Seaport Environmental facility in Redwood City for
disposal/recycling.
' Soil Vapor Extraction System Operation and Maintenance
During the third quarter of 2012 the SVE system and associated infrastructure were installed at
' the subject site. On August 29, 2012, the system was started-up and the initial source test was
conducted on the triple-nested soil vapor extraction wells VEW 1-S, VEW 1-I and VEW 1-D. The
system ran intermittently during the reminder of the 2012 year. The SVE system operates by
' extracting hydrocarbon vapors from the subsurface through wells VEWl-S/IID, which are then
destroyed within the SVE system by catalytic oxidation, before being discharged through the
stack. The results of the SVE startup and initial source testing were discussed in Ground Zero's
' Source Test Results for Soil Vapor Extraction System dated September 10, 2012.
During the first half of 2013, the SVE system operated from January 1 until January 14 when the
' system shut down because the blower motor and heating elements failed. Consequently, a new
blower motor was installed and the replacement heating elements were custom fabricated and the
system was restarted on May 13, 2013. After the restart, the system was operational until late June,
when it was shut down for additional troubleshooting and repairs. The system was restarted on July
2, 2013, after the heating element was re-wired and it operated until August 7, 2013, when the
system shut down because the heating elements failed. The system was repaired and restarted on
G9Data GROUNDZEIWOOLSEY.ROB\Reports\QMRU015 QMRs\AMUAL REPORT\Final Textjw.doc
