All work under this permit must <br /> meet with the approval of the State Representative. <br /> Failure on the Permittee's part to comply with any provision will be cause for revocation <br /> of this permit. <br /> IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL FILL <br /> OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY GRANTOR. <br /> Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State <br /> Standard Specifications Reference to Engineer in the State Standard Specifications <br /> shall include State Representative. <br /> Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, <br /> and Business and Professions Code, Section 8771. Permittee shall physically inspect <br /> the work site and locate survey monuments prior to work commencement. Monuments <br /> shall be referenced or reset in accordance with Business and Professions Code. <br /> A pre-job conference with the Permittee, Contractor, and State Representative may be <br /> required prior to start of work. Permittee shall contact the State Representative and <br /> arrange the meeting. This meeting may be waived at the discretion of the State <br /> Representative. <br /> Notwithstanding General Provision #4, your contractor is required to apply for and <br /> obtain an encroachment permit prior to start of work. <br /> Permittee shall, prior to commencement of any work, provide the State Representative <br /> with the name and phone number of the person in responsible charge of the work to <br /> be performed under this permit. <br /> This permit is not valid until Permittee has obtained permission from adjacent property <br /> owners, Irrigation Districts, Cities, Counties or other interested parties to perform the <br /> proposed work. <br /> All work shall be performed in accordance with the current Department of <br /> Transportation Standard Specifications and the Department of Transportation <br /> Encroachment Permit Utility Provision dated August 1998. <br /> In addition to the attached General Provisions, the following special provisions are also <br /> applicable for installation and monitoring of groundwater monitoring wells: <br /> 1. Permittee shall furnish, place and maintain required signs, safety equipment, traffic <br /> control, and warning devices in accordance with the manual of Traffic Controls <br /> published by the Department of Transportation. <br /> 2. All personnel shall wear hard hats and orange vests, shirts or jackets as appropriate. <br /> 2 <br />
