Further investigation of the groundwater contamination at this site will be necessary. <br /> f The well receptor survey included in the QMR is incomplete. in addition to reviewing the Califomia <br /> Department of Water Resources records and performing a physical search of the area, PHSIEHD <br /> 4 microfiche records should be researched to locate potential receptors. All wells located within a 2,000 foot <br /> radius of the site should be plotted on an accurately scaled site map. Construction details for the well <br /> located at 705 E. Lodi Ave. should be provided to PHSIEHD. Septic systems and any other relevant <br /> conduits should also be identified and plotted on the site map. <br /> Petroleum contamination was evidenced in groundwater collected from Monitoring Well TT-2 during the <br /> June 2, 1999 sampling event. In the subsequent QM event on August 19, 1999 the concentrations of <br /> contaminants evidenced a decrease in TT-2. Continue quarterly monitoring of TT-2 for all constituents of <br /> concern to confirm contaminant concentrations. Provide PHSIEHD with arose gradient diagram for this <br /> site in the next QMR. <br /> r <br /> When data shows that groundwater contamination is decreasing or stabilizing then a closure report should <br /> N be submitted to PHSIEHD. The closure report should contain all information as outlined in the"Appendix <br /> B,Tri-Regional Board Staff Recommendations for Preliminary Investigation and Evaluation of <br /> Underground Tank Sites,No Further Action Request". Mass balance calculations for contaminants <br /> remaining in the soil and groundwater and fate-transport information that supports conclusions for site <br /> closure should be provided. <br /> Additional well survey information should be submitted to PHSIEHD by January 25,2000.
