Q <br />0• <br />11. 24 -Hour Reporting <br />The owner or operator shall report to the Department any <br />noncompliance which may endanger health or the environment. <br />Any information shall be provided verbally within 24 hours <br />from the time the owner or operator becomes aware of the <br />noncompliance. The following shall be included as <br />information which must be reported verbally within 24 hours <br />to the Toxic Substances Control Program, Region 1 office at <br />(916) 855-7708. <br />a. Information concerning a release of any hazardous <br />waste which may cause an endangerment to public <br />drinking water supplies. <br />b. Information concerning any release or discharge of <br />hazardous waste, or of fire or explosion from the <br />facility, which could threaten human health or the <br />environment outside the facility. The description of <br />the occurrence and its cause shall include: <br />(1) Name, address, and telephone number of the owner <br />or operator; <br />(2) Name, address, and telephone number of the <br />facility; <br />(3) Date, time, and type of incident; <br />(4) Name and quantity of material(s) involved; <br />(5) The extent of injuries, if any; <br />(6) An assessment of actual or potential hazard to <br />the environment and human health outside the <br />facility, where this is applicable; and <br />(7) Estimated quantity and disposition of recovered <br />material that resulted from the incident. <br />A written submission shall also be provided within <br />five days of the time the owner or operator becomes <br />aware of the circumstances. The written submission <br />shall contain a description of the noncompliance and <br />its cause; the periods of noncompliance (including <br />exact dates and times), and if the noncompliance has <br />not been corrected, the anticipated time it is <br />expected to continue; and steps taken or planned to <br />reduce, eliminate, and prevent reoccurrence of the <br />noncompliance. <br />12. Other Noncompliance <br />The owner or operator shall report all other instances of <br />noncompliance not otherwise required to be reported at the <br />aC <br />
